Patient Registration

If you would like to register with the practice, please go here to ensure that you live within the practice area.

You can collect the relevant registration forms from the surgery or download them from this page.

Please complete separate forms for each person who wishes to register and bring them into reception.

Please inform us if you have any information or communication needs upon registration or whilst a patient at our Practice.

Please note, upon receipt of your registration forms, it will take 2 working days to process and for it to be on our system.

You can register as a new patient by completing our online form. After you have completed the form you can submit it directly to us or print it off ready to bring in to the surgery.

By completing and sending this form, this does not mean you are registered as a patient at The Saville Medical Group. We will require you to sign the form and provide proof of identity when you attend the surgery for the first time.

Proof of Identity

You must provide proof of identity and also proof of address when registering. This can be any of the following formats and must be dated within three months:
  • Utility Bill
  • Bank statement
  • Rent Book
  • Council tax statement
  • Passport
  • Driving License

Temporary Registration

If you are ill while you are away from home or if you are not registered with a doctor but need to see someone to receive emergency treatment from a local GP, you can register as a temporary patient.

You can be registered temporarily for up to 3 months. This process means that you will be temporary registered at our surgery but will also remain a permanent patient at your GP. After that time, you will have to re-register as a temporary patient or permanently register with the practice.

To register as a temporary patient, simply come into the surgery and fill in a temporary resident form. It will be processed as soon as possible and you will then be able to make an appointment or speak to a Clinician. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
